Please use this COST Referral form to refer students to who meet the indicated criteria to University High School’s Coordination of Services Team (COST).  COST meets weekly and refers students to a variety of intervention programs, including, but not limited to:

·       Individual Counseling

·       Family Counseling

·       Group Counseling/Support Groups

·       School Psychologist

·       Tutoring/Mentoring Programs

·       Student Success Team

·       Attendance Monitoring

Guidance Counselors, Group Counseling Facilitators, Deans, our School Psychologist, our Attendance Counselor, our Nurse, our UCLA Coordinator and our Psychiatric Social Worker are all a part of COST. 

When you refer a student to COST, all support providers share their knowledge of the student.   This allows us to quickly create a student profile, assess the student’s most immediate needs, and determine the most appropriate intervention for the student.  Completed Intervention Referral Forms provide us with the information the Team needs to do their job.  Please take the time to fill out the referral forms and record the classroom interventions that you have already taken.

Once you have referred a student to COST, the team will notify you, within two weeks, of the programs/actions the team is pursuing with this student.
